Claim Form Login

If you are a Class Member and you want to get a payment, you must complete and submit a Claim Form by June 30, 2025. THE CLAIM FORM MUST BE SIGNED AND MEET ALL CONDITIONS OF THE SETTLEMENT AGREEMENT.

The Settlement Administrator will review your Claim Form. If accepted, you will receive a share of the Settlement Fund. This process takes time, please be patient.

To start your online claim, you can find your Unique ID & PIN printed on the Notice that was sent to you. If you are unable to locate your ID & PIN, please contact the Settlement Administrator at info@SPLLCSettlement.com or (833) 296-0885. Please request your Unique ID & PIN by providing the Class Member’s full name and mailing address.
